MDS Coordinator

Join Our Team as an MDS Coordinator!

We are seeking an experienced and detail-oriented MDS Coordinator to oversee the Minimum Data Set (MDS) assessment process and ensure compliance with federal and state regulations. This role is responsible for coordinating resident assessments, developing individualized care plans, and supporting reimbursement accuracy through effective case management and documentation.

Key Responsibilities

  • Complete and ensure accuracy of MDS assessments for all residents.

  • Develop comprehensive care plans based on resident needs and assessment findings.

  • Review admissions, conduct resident assessments, and collaborate with the Admissions Coordinator on placement recommendations.

  • Monitor Medicare and Medicaid documentation, assessment schedules, and reimbursement-related data.

  • Track Case Mix Index (CMI) scores and identify potential reimbursement risks.

  • Maintain current knowledge of MDS 3.0, RAI, Quality Measures (QM), and regulatory changes.

  • Collaborate with physicians, nursing staff, residents, and families to support quality care outcomes.

  • Serve as a resource for clinical staff regarding Medicare criteria and regulatory requirements.

Qualifications

  • Bachelor's degree in Nursing required.

  • Current and valid state Registered Nurse (RN) license.

  • Knowledge of MDS 3.0 assessments, Medicare/Medicaid processes, and long-term care regulations.

  • Previous clinical nursing experience required; MDS Coordinator experience preferred.

  • Strong analytical, data collection, reporting, and problem-solving skills.

  • Proficiency in medical terminology and healthcare documentation.
