A Brief Overview
The Clinical Pharmacist at SVH provides comprehensive pharmaceutical care to patients birth to death by optimizing medication therapy, providing disease state prevention and management and promoting health and wellness. This mission is accomplished by upholding the American College of Clinical Pharmacist philosophy of pharmaceutical care; blending a caring orientation with specialized therapeutic knowledge, experience and judgment for the purpose of ensuring optimal patient outcomes. The Clinical Pharmacist also contributes to the generation of new knowledge that advances health and quality of life. This is a fully remote position.
Education Qualifications

  • Bachelor's Degree Of Science in Pharmacy from an accredited school of pharmacy Required or
  • Doctorate PharmD from an accredited school of pharmacy Required


Experience Qualifications

  • Successful completion of an ASHP accredited PGY1 plus one year of work in healthcare system or relevant clinical experience. Required or
  • Completed equivalent clinical fellowship Required or
  • 3 years Work experience that includes essential duties at healthcare system Required


Skills and Abilities

  • Medical terminology and knowledge of medication names. Complete 20 CE hours annually in area of expertise.
  • Ability to apply mathematical concepts and apply mathematical operations to such tasks as analysis of variance, correlation techniques, sampling theory, and factor analysis. Also, use of spreadsheets, internet/e-mail, database software, contact management, and the electronic medical record.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals. (Required proficiency)
  • Excellent customer service skills and ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.


Licenses and Certifications

  • Pharmacist - KBP Must be licensed in Kansas within 90 days of hire. Required
  • Board Certified Pharmacy Specialist (BCPS) - BPS Required for the first time within 2 years of hire however, it is not required to be renewed Required
  • ASHP Certification within 2 years of hire is required or clinical certification from an accredited organization or board certificate with 2 years of hire. Required


What you will do

  • Review, interpret and verify provider medication orders making any recommendations and clinical interventions as necessary based on patient specific conditions, disease states, age and other clinical factors such as safety and cost effectiveness. Complete assigned pharmacy consults proficiently and following all pharmacy policies and procedures.
  • Assess the status of the patient's health problems as possible and determine whether the prescribed medications are optimally meeting the patient's needs and goals of care. Recognize untreated health problems that could be improved or resolved with appropriate medication therapy. Follow the patient's progress to determine the effects of the patient's medications.
  • Lead, co-lead or participate in hospital/pharmacy committees remotely upon assignment by clinical coordinator/ manager or pharmacy director according to the specialty area covered such as antimicrobial stewardship, opioid stewardship, anticoagulation committee.
  • Educate and train pharmacy personnel/students/residents and other healthcare professionals remotely, and exhibit qualities of servant leadership.
  • Participate in at least 50% of assigned schedule engaged in patient care planning within a multidisciplinary team in respective areas remotely. Provide a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient's medication use.
  • Support the health care team's efforts to educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization. Advise the patient on how to best take his or her medications.
  • Participate in original research, quality improvement projects, or scholarly activities such as posters or presentations remotely.
  • Contribute to the development and refinement of specialty-specific quality dashboards and performance reports.
  • Maintain active involvement in professional pharmacy organizations which needed for their specialty/preceptor/instructor position (such as ACCP, ASHP) remotely.
