Your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments to identify the emotional, mental, and social needs of seniors, facilitating a deeper understanding of each client's unique situation.
• Collaborating with clients, families, and caregivers to develop and implement individualized care plans tailored to specific goals, preferences, and cultural contexts.
• Providing ongoing counseling and emotional support, enabling seniors to navigate transitions such as health declines, loss, or relocation.
