Empower Well Therapy is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team, specializing in hospice care services. This position is primarily remote, allowing for flexibility in your work schedule while you provide essential services to families in need. Collaborating with a multidisciplinary team, you will engage in remote consultations, assessments, and therapy sessions, making use of digital platforms designed for telehealth. Your ability to adapt to remote working conditions will be key in delivering effective social support.

• Conduct comprehensive evaluations and assessments of clients' psychological, emotional, and developmental needs.
• Contribute actively to the formulation and implementation of Individualized Family Service Plans (IFSPs), ensuring tailored support that aligns with each family's circumstances.
• Monitor client progress and outcomes by utilizing evidence-based practices and maintaining precise documentation according to regulatory standards.
• Develop and maintain rapport with children, families, and team members to promote a supportive and nurturing environment conducive to healing and development.
• Provide training and resources to families, enabling them to effectively support their child's development, while respecting their values and preferences.
• Embrace and apply the principles of Natural Learning Environment Practices in all family interactions.
• Ensure compliance with all applicable federal, state, and local regulations to safeguard the health and safety of all clients and staff.

As an LCSW at Empower Well Therapy, your work will primarily focus on families in the hospice setting, navigating the complexities of care for loved ones at the end of life. You will support not only children but also their associated caregivers, empowering them to cope with grief, transitions, and the emotional aspects of this challenging time. You will work closely with families to enhance their coping mechanisms and emotional resilience.

• A Master's Degree in Social Work (MSW) or a Doctorate in Social Work (DSW) is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Illinois Department of Public Health.
• Possession of a valid driver's license is necessary for potential local travel.
• Demonstrated experience utilizing effective strategies to adapt to various learning styles and needs, particularly when working with children in early intervention settings.
• Commitment to upholding the principles and practices of Natural Learning Environment is critical.
• Excellent organizational skills coupled with time management proficiency.
• Strong interpersonal communication skills to foster collaboration with team members and families alike.

• At least one year of experience in social work or related fields focusing on child development and early intervention scenarios, particularly within hospice environments.

The salary for this position ranges from $75,000 to $80,000 annually, commensurate with experience and qualifications.
