Position Summary
* : This is an in-person position. Candidates must be able to work onsite and should be comfortable commuting to our office. Remote or hybrid work is not available for this role.
The Medical Records Custodian is responsible for the maintenance, protection, and authorized release of patient health information in accordance with all applicable legal, regulatory, and company standards. This role ensures the accurate and timely processing of medical records requests while maintaining strict compliance with HIPAA regulations and patient confidentiality requirements.
Primary Responsibilities
· Receive, review, and process incoming first-party and third-party medical records requests.
· Maintain accurate electronic records and filing systems to support efficient retrieval, auditing, and regulatory compliance.
· Evaluate requests for completeness, accuracy, and proper authorization prior to release.
· Review and process objections, certifications, subpoenas, and other legal documentation associated with third-party records requests.
· Fulfill medical records requests within established timelines and regulatory deadlines.
· Ensure all records disclosures comply with HIPAA, applicable federal and state regulations, and company privacy policies.
· Maintain the confidentiality, integrity, and security of protected health information (PHI) at all times.
· Accurately track, document, and log all records requests, disclosures, and related correspondence for auditing and compliance purposes.
· Prepare, print, assemble, and mail medical records, certifications, and supporting documentation as required.
· Communicate professionally with patients, attorneys, healthcare providers, insurance representatives, and internal teams regarding records requests and documentation needs.
· Identify and escalate incomplete, inaccurate, or potentially non-compliant requests when necessary.
Qualifications
· High school diploma or equivalent required
· One or more years of experience or formal training in medical records, healthcare administration, legal support, or a related field preferred.
· Working knowledge of Health Insurance Portability and Accountability Act (HIPAA) and patient privacy regulations.
· Strong attention to detail and organizational skills.
· Ability to handle sensitive and confidential information with discretion.
· Proficiency with electronic medical record (EMR) systems, document management systems, and Microsoft Office applications preferred.
· Strong written and verbal communication skills.
· Ability to manage multiple deadlines in a fast-paced environment.
