Job description

We're hiring an experienced Clinical/Infusion Pharmacist for a full-time, direct hire opportunity with a rapidly growing home infusion pharmacy in Syracuse, NY. If you're passionate about patient care, sterile compounding, and working in a collaborative clinical environment, this is an opportunity you won't want to miss!
Position Details
Location: Syracuse, NY
Job Type: Direct Hire
Start Date: ASAP
Schedule: Monday - Friday | 8:00 AM - 4:30 PM
On-Call: Participation in rotating on-call schedule required
Key Responsibilities Prepare and verify sterile compounded IV medications in accordance with USP standards and regulatory guidelines
Monitor patient therapies, including pharmacokinetic dosing and TPN management
Review laboratory values and provide clinical recommendations when appropriate
Educate patients and caregivers on infusion therapies, infusion pumps, sterile technique, and catheter care
Collaborate with physicians, nurses, intake, reimbursement, and pharmacy operations to ensure seamless patient care
Maintain compliance with Board of Pharmacy regulations, HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards
Assist with inventory management, refrigerated medications, and medication preparation for home delivery
Qualifications Active New York Pharmacist License in good standing
2+ years of home infusion or specialty infusion pharmacy experience
Strong sterile compounding and clinical monitoring experience
IVIG experience is a plus
Knowledge of specialty pharmacy reimbursement, payer requirements, and contract obligations preferred
Experience with CareTend is a plus
