Frontier Living provides services and support for members of our communities who need help in living independently. We are on a mission to provide high-quality support and care to individuals, and we are seeking a capable and dedicated Designator Manager to lead our program coordination, evaluation, and oversight efforts.
Job Description:

As the Designator Manager at Frontier Living, you will play a critical role in ensuring the success and compliance of our programs. Your responsibilities will encompass program coordination, evaluation, and oversight, as mandated by state regulations.
Key Responsibilities:

1. Program Coordination and Evaluation:

o Coordinate service delivery for each person served by the program, in accordance with state regulations.
o Oversee program management and conduct evaluations to ensure program quality and continuous improvement of services provided by Frontier Living.
2. Designator Manager Responsibilities:

o Maintain a current understanding of licensing requirements to ensure program compliance.
o Ensure that the designated coordinator fulfills their responsibilities, as specified in 245D.081 State Statues.
o Implement corrective actions as necessary, following the review of incident and emergency reports.
o Conduct internal reviews of incident reports related to maltreatment, as required by regulations.
o Evaluate and ensure the satisfaction of individuals served, their legal representatives, and case managers regarding service delivery and progress toward achieving outcomes.
o Protect the rights of each person served, as identified in relevant regulations.
o Ensure that staff competency requirements are met, including orientation and training, as outlined in regulations.
o Take corrective action when ordered by the commissioner and ensure compliance with license terms and variances.
o Evaluate information to develop, document, and implement ongoing program improvements.
Qualifications:

  • Bachelor's degree in a field related to human services or equivalent work experience.
  • A minimum of three years of supervisory level experience in a program providing direct support services to persons with disabilities or persons aged 65 and older.
  • Competency to perform the designated managerial duties, as specified in state regulations.
  • Knowledge of state regulations and licensing requirements relevant to the 245D Waiver program in the State of Minnesota.
