Job description

Patient Care Coordinator

As a Patient Care Coordinator, you will:

Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services available in a market.

Conduct facility visits at the physicians' request to assist program clinical team in determining eligibility.

Collaborate with program staff and other field sales resources via weekly meetings. Actively participate in weekly program business development meetings.

Assist program in processing of physician orders as directed.

You will report to a Market Executive of Sales.

Required/Preferred Experience/Skills:

RN/LPN/LVN license is required.

Bachelor's of Science in Nursing preferred.

3 years clinical experience preferred.

Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.

Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.

Travel within an assigned territory and to sales meetings as required.

This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year
