Under the supervision of the Director of Case Management, you will participate as the patient program manager of the outpatient / inpatient team of professionals to meet discharge goals of patients and families, and monitor outcomes and utilization of services. As the RN Case Manager / MSW Case Manager, you will also use your effective communication skills by communicating with payor representatives and providing age-appropriate clinical intervention to meet psycho-social and referral needs of identified patients and families. The work you do makes a tremendous impact on our patients and the community!

Position: Case Manager
Shift: Monday - Friday; Day Shift
Experience: 
1. Minimum of two years recent experience in a hospital or rehabilitation facility required. 
2. Minimum of two years recent insurance, case management experience preferred.
Education: 
MSW or RN required. 
Licensures/Certifications: 
CCM preferred.
