Job description

Registered Nurse - Case Manager

Salary Range: $65,000 annually

Provides health services to the youth of Hope. Collaborates with other youth service lines to ensure optimal care for all youth. Communicates health updates to the parents/guardians of the youth. Manages youth's medications and treatments in the residential and school setting. Provides ongoing health services training to the residential and school staff.

Organizes, coordinates, and provides care coordination to youth within case assignment. Receives and implements provider's orders. Monitors provider's orders on a continuous basis. Serves as a liaison between youth, staff, and provider. Reads and responds to all communication regarding youth's medical care. Collaborates with providers and all members of the youth's team to ensure the best trajectory for the youth in the RN Case Manager's case assignment. Performs bi-annual physical assessments on all youth within RN Case Manager's case assignment. Schedules and attends youth's medical appointments when appropriate. Audits medication closets in residential for proper function and safety to include: Verification of narcotic counts. Reordering of medications as needed. Organizing medication closet. Reviewing all medication documentation for completeness and accuracy. Re-educate staff on processes as needed. Attends all meetings pertaining to youth within RN Case Manager's case assignment (i.e.: Plex Leadership, Whole Plex, IEPs, staffings, clinical reviews, etc.). Performs all duties within the Licensed Practical Nurse job description.

Maintains professional relationships and boundaries with team members, youth, and youth's families. Identifies and reports potential safety hazards. Assumes responsibility as a mandated reporter concerning incidents of alleged abuse and/or neglect of individuals served by Hope, as outlined in the agency's policies and procedures. Performs all other duties as required or assigned.

Work is performed primarily in an indoor office, school nurse office, or residential home, free from extreme weather conditions. Work will include walking, direct care of the youth, and the requirement to drive. Work may include a moderate amount of noise (business office with computers, phone and printers). Work will require working flexible hours and on-call hours.

Ability to assess and refer youth for higher level of care when needed. Ability to listen to and understand information and ideas presented through spoken words and sentences. Ability to communicate and deliver staff education clearly and effectively, both orally and in writing; Ability to navigate and utilize various software systems such as Microsoft Office and ERP systems. Ability to plan and manage multiple tasks to meet specific deadlines. Ability to handle confidential information with discretion. Ability to analyze and prepare documents, reports and correspondence. Ability to exercise sound judgement in making critical decisions. Ability to handle difficult and stressful situations with professional composure. Ability to establish, develop and maintain cooperative relationships with co-workers. Skill in completing assignments accurately and with attention to detail.

Graduate of an accredited nursing program. Current, unencumbered license to practice as a Professional Registered Nurse in the state of Illinois. Minimum of one-year experience as a registered nurse.

2 plus years previous experience with pediatric population. 2 plus years previous experience with persons with developmental disabilities.
