The Director of Case Management provides strategic and operational leadership for Case Management, including Nursing Care Transition and Social Work functions across SMH. This leader is responsible for ensuring effective care coordination across the continuum, optimizing patient throughput, supporting quality outcomes, regulatory compliance, and financial stewardship while promoting exceptional patient and family experiences. The Director partners closely with physician leadership, nursing, finance, patient access, revenue cycle, quality, behavioral health, post-acute providers, and community organizations to develop an integrated care management strategy that supports organizational goals for clinical quality, patient safety, length of stay, readmission reduction, and value-based care.

Qualifications

Education:

         BSN required.

         Master's degree in Nursing, Business or Healthcare Administration required.

Licensure/Certification/Registry:

         Current licensure in the State of Illinois as a Registered Professional Nurse.

         National nursing certification held or achieved within one year of hire.

         Certified as an Accredited Case Manager (ACM) or Certified Case Manager (CCM) preferred Experience:

         Minimum five years nursing leadership/administrative experience required.

         7-10 years of experience, with demonstrated success in departmental leadership

         Clinical nursing experience in a tertiary care practice environment required.

         Progressive experience in case management, care coordination, utilization management, transitions of care, and discharge planning within an acute care setting preferred.

         Experience leading interdisciplinary teams that include nursing, social work, and care coordination functions preferred.Other Knowledge/Skills/Abilities:

         Evidence of knowledge and successful application of project management, change implementation, and quality/process improvement skills.

         Evidence of knowledge and successful application of national and state regulatory requirements.

         Evidence of facilitating the professional growth and development of staff and nurse leaders.

         Demonstrates:

a.       Outstanding human relations/communication skills.

b.       Ability to bring complex issues to closure.

c.        Understanding/experience with budgeting/fiscal management activities.

         Ability to function as a change agent.
