Position Summary

As a Hospice and Home Health CNA, you will provide compassionate, hands-on care to patients in their homes, supporting their physical, emotional, and dignity needs. You will play a vital role on an interdisciplinary care team, serving individuals with chronic conditions, recovery needs, or end-of-life care.

Key Responsibilities
  • Provide personal care including bathing, grooming, dressing, and toileting
  • Assist with mobility, transfers, and safe ambulation
  • Monitor and report changes in patient condition to the clinical team
  • Support patients with activities of daily living (ADLs)
  • Offer companionship and emotional support to patients and families
  • Maintain accurate documentation per agency and regulatory standards
  • Follow individualized care plans under supervision of licensed nursing staff
  • Adhere to infection control, safety, and hospice care protocols
Qualifications
  • Current Oregon Certified Nursing Assistant (CNA) certification
  • Previous experience in home health or hospice preferred, but not required
  • Ability to work independently and manage time effectively
  • Strong communication and interpersonal skills
  • Compassionate, patient-centered approach to care
  • Valid driver’s license, reliable transportation, and insurance
