Job description

  • JOB SUMMARY
    • The Medication Management Clinic (MMC) is an ambulatory care pharmacist led clinic that partners with primary care and specialty physicians to assist patients navigating the pharmacy system, including comprehensive medication management, education, insurance authorizations, refills, and financial assistance. MMC pharmacists work at the top of their license through collaborative practice agreements to manage drug therapy including prescriptive authority for various disease states, such as: diabetes, hypertension, hyperlipidemia, asthma, COPD, weight loss, tobacco cessation, rheumatologic indications, irritable bowel syndromes, and hepatitis C. The MMC is a hospital-based clinic that is rapidly expanding with the addition of new providers and disease states. The clinic is embedded within physician clinics as well as additional office locations in the outpatient pharmacy and hospital. Pharmacists work independently with their patients to meet therapeutic goals while also being a vital member of a multidisciplinary team. As the MMC grows, pharmacists have opportunities to contribute to the development of new services and find a role within the clinic that best suits them.
  • JOB REQUIREMENTS
    • Minimum Education
      • B.S. Pharmacy or Pharm D, or Master of Pharmacy
      • TSBP Preceptor required within 12 months of hire
    • Minimum Work Experience
      • Previous hospital experience with clinical responsibilities
    • Required Licenses/Certifications
      • Active registration as a licensed pharmacist in the state of Texas
      • BLS
    • Required Skills, Knowledge, and Abilities
      • Assess, document, and perform physician consultations, excluding total parenteral nutrition, but including all medication dosing consults
      • Perform home medication reconciliation
      • Provide education presentations to pharmacy staff and other staff upon request
      • Ability to effectively direct and/or supervise personnel as appropriate
      • Ability to handle confidential information
      • Ability to maintain professional appearance and decorum at all times
      • Ability to demonstrate excellent oral and written communication skills
