How you’ll make an impact in this role
  • Coordinate the overall interdisciplinary plan of care from patient admission to discharge, integrating patient/family preferences and values.
  • Monitor patient care through assessments, evaluations, and review of patient records.
  • Advocate for necessary resources and the removal of patient care barriers.
  • Maintain ongoing communication with supervisors and health providers to ensure effective implementation of the health plan.
  • Act as a resource for adequate medical record documentation, appropriate services, and post-discharge care options.
What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Required Credential(s):
    • Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job
    transfer date.
  • Preferred Credential(s):
    • BLS Provider. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of
    hire.
