JOB SUMMARY:

A Licensed Practical Nurse or Registered Nurse assists the Clinical Manager/Administrator with the coordination of home health services to patients in their place of residence in accordance with physician orders and strives to provide the highest quality of care. The Clinical Manager Assistant is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the assignment of personnel.

QUALIFICATIONS:

1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.

2. Three to five (3-5) years of experience in nursing, preferred.

3. One (1) year experience in home health nursing, preferred.

4. Acceptance of philosophy and goals of Agency.

5. Ability to exercise initiative and independent judgment.

6. Ability to work with individuals to enlist the cooperation of many people to perform/achieve a common goal.

RESPONSIBILITIES:

1. Understands and adheres to established Agency policies and procedures.

2. Participates in the development of standards that ensure safe and therapeutically effective service to patients/families.

3. Consults with the Clinical Manager/Administrator in determining a staffing pattern that will accomplish stated objectives and promote a maximum level of utilization of health personnel.

4. Assists in recruiting nursing personnel.

5. Responsible for assisting in the orientation of new patient care personnel.

6. Assists in the planning and consultative needs of staff.

7. Assists in the implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state and local laws.

8. Displays a willingness to support policies and procedures and uses appropriate channels for changes of such policies.

9. Assists in the coordination of interdisciplinary team services.

10. Assists in the organization of nursing services to provide safe and therapeutically effective service.

11. Participates in the coordination of the Agency’s services with the services of other community agencies.

12. Participates in studies and research and other administrative functions as assigned.

13. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performances.

14. Observes confidentiality and safeguards all patient-related information.

15. Accepts responsibility for regular attendance and punctuality; fulfills job-related requirements without regard to the time involved.

16. Serves as a resource person to employees.

17. Develops a cooperative relationship and communicates effectively and professionally with physicians and other healthcare providers.

18. Reports any problem relating to patient care and/or employee well-being.

19. Immediately reports any accident, incident, lost articles, or unusual occurrence to the Clinical Manager/Administrator.

20. Attends pertinent continuing education programs other than routine in-services and shares information with staff.

21. Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.

22. Assists with the Agency’s ongoing Quality Assessment and Performance Improvement (QAPI) program.

23. Assists with the implementation and evaluation of patient plans of care.

24. Reviews patient medical records for compliance with federal, state, accrediting bodies, local, and Agency policies and guidelines.

25. Assists Clinical Manager in conducting patient care conferences, staff meetings and maintains documentation. Participates in community education projects.

26. Ensures that all necessary supplies and equipment are available.

27. Must be able to perform nursing functions in times of staff shortage.

28. Available to share administrative and clinical on-call with nursing personnel as needed/permitted by applicable regulations.

29. Ensures that documentation is completed in accordance with Agency policy.

30. Assists with staffing visit assignments and on-call assignments.

31. Other duties as assigned by the Clinical Manager/Administrator.

32. Coordinates referral, intake, and authorization process under the supervision of the Clinical Manager/Administrator.

33. Other duties as assigned.

WORKING ENVIRONMENT:

Works indoors in Agency office and patient homes and travels to/from patient homes.

LIFTING REQUIREMENTS:

Ability to perform the following tasks if necessary:

• Ability to participate in physical activity.

• Ability to work for an extended period of time while standing and being involved in physical activity.

• Moderate lifting.

• Ability to do extensive bending, lifting, and standing on a regular basis.
