Location: Saratoga Springs, NY
Employment Type: Full-time

Salary Range: LPN - $26.00 - $39.00 hourly, based on experience and qualifications
BSW- $21.55 - $35.38 hourly, based on experience and qualifications

About the Role
We're looking for a dedicated Care Management Assistant to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you'll be a vital part of our administrative team, ensuring excellent service, collaboration, and patient outcomes in a fast-paced healthcare environment. +ADD SUMMARY OF POSITION
What You'll Do
  • Support RN Case Managers/LMSW in meeting with patient and/or family/personal representative within 48 hours of admission (excluding weekends/holidays), to evaluate, and identify discharge needs in compliance with established guidelines and policies. Provides support and information, as needed.
  • Assists with discharge planning activities to support discharge timely and efficiently through collaboration with providers and multi-disciplinary team.
  • Works to minimize discharge delays and achieve appropriate discharge times.
  • Ensure adherence to confidentiality and privacy policies
  • Supports timely community resource referrals, in alignment with patients' benefits, within EPIC as needed based on patient choice and post-hospitalization needs for discharge and transfer.
  • Provides appropriate linkages, referral coordination, and follow-up.
  • Collaborate with community liaisons and health care partners to facilitate optimal discharge plans.
  • Advocates for the patient's and family's needs.
  • Supports delivering copy of regulatory notices to patients as directed by the RN Case Manager or RN Utilization Manager
  • Issues "Important Medicare Message" (IM) as required 24-48 hours prior to discharges of Medicare and/or Managed Medicare patients.
  • Issues Discharge Notice for Non-Medicare Patients as required within 24 hours to discharge for Commercial, Medicaid and Non-Medicare Patients.
  • Deliver compassionate, patient-centered care in alignment with Saratoga Hospital values
  • Collaborate with interdisciplinary teams to support health, healing, and service excellence
  • Perform administrative responsibilities with a focus on safety, quality, and efficiency
  • Use hospital systems and tools to document care and support operations
  • Continuously seek opportunities to improve processes and support patient and staff satisfaction
  • Serve as a positive, professional representative of our hospital and community

What You Bring
  • Minimum of 2 years Acute Care Experience.
    Knowledgeable of New York State and Federal Entitlement Programs and regulatory requirements.
  • Current Unrestricted LPN license in NY State or Social Work Degree (BSW)
  • Strong communication and teamwork skills
  • Commitment to providing patient-first, high-quality service
  • Comfort working in a fast-paced, collaborative environment
