What You Will Do:

Coordinate the processes of assessment, planning, implementation, and evaluation of patient care in collaboration with patients, families, physicians, and support services.

  • Conduct comprehensive and ongoing physical and psychosocial assessments, including functional and nutritional screenings.
  • Use evidence-based assessment tools and analytical models to identify patterns and determine nursing diagnoses.
  • Collaborate with interdisciplinary teams to develop individualized plans of care with clearly defined interventions and outcomes.
  • Implement care interventions, including treatments, medications, and patient/family education, ensuring safe and timely delivery.
  • Delegate and coordinate patient care with support services while maintaining oversight and accountability.
  • Evaluate effectiveness of interventions and revise care plans based on reassessments.
  • Actively involve patients and families in care decisions and discharge planning.
  • Maintain thorough documentation and adhere to nursing practice standards, safety regulations, and compliance requirements.
