Position Summary
This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.
The Licensed Vocational Nurse, (LVN) provides nursing care to patients within the LVN scope of practice as delegated by the Registered Nurse, (RN). The LVN performs tasks such as taking vital signs, administering medications, assisting with patient hygiene, and documenting patient care under the supervision of a Registered Nurse (RN) and in accordance with the LVN scope of practice.
Responsibilities include directing unlicensed employee activity, collecting patient information, performing focused patient assessments, assisting the RN in planning and implementing interventions/MD orders, assisting the RN with teaching patients/family, and evaluating the care rendered according to the LVN scope of practice. The LVN must effectively communicate and document data relevant to the care of the patient. The LVN utilizes knowledge of patient age, cultural diversity, and psychosocial and/or spiritual needs into the patient's care to support the RN and the nursing process. The LVN collaborates with other professional disciplines to ensure safe and effective patient care delivery and the achievement of desired patient outcome.
Provides nursing care within the scope of the LVN Practice.

  • Collects data and uses scope of practice to appropriately deliver interventions and when necessary to escalate to the RN or MD. (Utilizes Chain of Command)

  • Collaborates with licensed and unlicensed personnel.
  • Demonstrates knowledge of the clinical information system and applies principles to patient documentation.
  • Follows a teaching plan and evaluates effectiveness of learning in collaboration with the RN; family is included in teaching plan as necessary.
  • Serves as a role model and mentors new staff members.
  • Demonstrates the ability to integrate knowledge of job specific competencies.
  • Identifies, develops and implements an action plan for self-development/professional growth.
  • Identifies subtle as well as obvious patient problems and reports to an appropriate resource person considering multidisciplinary team members and/or Chain of Command.

Minimum Requirements
Experience
  • One year of experience in an Acute Care, Long Term Care, or Skilled Nursing Facility setting required

Education
  • Completion of an accredited LVN program required
  • Active California Board of Vocation Nurse License, LVN License required
  • American Heart Association BLS required
  • Intravenous Therapy and Blood Withdrawal (IVBW) Certification on LVN license required
