Job description

Resolution Think, through its joint venture partnership, Cognito Systems, LLC, are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to join our Referral Management Center team at Dover Air Force Base. The ideal candidate will support referral management, clinical-administrative review, documentation, and patient education. This role operates in an administrative capacity and does not involve routine hands on patient care.
Location: Dover Air Force Base, Delaware
Schedule: Monday - Friday 7:30a-4:30p
Travel: None
Key Duties:
  • Track and follow up on outstanding referral reports and documentation.
  • Ensure completed referrals from network and VA providers are returned to the referring provider within required timeframes.
  • Review referrals requiring nursing judgment and maintain accurate referral statuses and documentation.
  • Support specialty referral and recapture initiatives.
  • Educate patients and MTF staff on referral requirements, documentation standards, and timelines.
  • Maintain accurate and timely records in required healthcare systems.
Minimum Requirements:
  • Must be a US Citizen per contract requirements
  • Education: Graduate of an accredited practical or vocational nursing program.
  • Licensure: Current, active, unrestricted LPN/LVN license issued by any U.S. state, the District of Columbia, or a U.S. territory.
  • Certification: Current Basic Life Support certification (AHA or Red Cross)
  • Experience: One year of practical or vocational nursing experience. Referral management, utilization management, case management, or ambulatory care preferred.
    • Demonstrated computer proficiency and keyboarding sufficient to sustain electronic health record workflow.
    • Ability to read, write, speak, and understand English fluently.
