Job description

The Licensed Practical Nurse (LPN) is an active participant in the health care team who provides direct and indirect care to patients/families/legal guardians. The Licensed Practical Nurse (LPN) works collaboratively as a member of the multi-disciplinary treatment team and contributes to each patient’s individual treatment plan by helping develop, implement, and document appropriate goals and interventions, informed by patient strengths and presenting psychiatric problems. The principal functions of the position identified shall not be considered as a complete description of all the work requirements and expectations that may be inherent in the position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.

The following are duties and responsibilities that the LPN shall provide:

  • Maintains and follows nursing policies and procedures that conform to current standards of nursing practice, facility, philosophy, and operational policies
  • Complete health assessments, implement treatment plans, and accurately document outcomes in the EHR for all assigned clients each shift in a timely manner
  • Reviews daily incident reports and records patient incidents daily
  • Assist in admissions and discharges while abiding by facility nursing procedures
  • Follow physician orders as written in the treatment plan, monitoring withdrawal based on CIWA, COWS, or CINA
  • Assess and report signs and symptoms indicating physiologic and or psychosocial changes patient’s condition to the provider and clinical team
  • Assist with referrals to appropriate health agencies and coordinate medical services
  • Providing health education on a variety of topics to clients, including but not limited to general health care, withdrawal management, medications, diabetes, safe sex, dual diagnosis, and nutrition in a way they understand
  • Observe and participate in medication procedures and make recommendations as needed
  • Attend regular staff meetings, trainings, and reviews with the nursing team
  • Maintain compliance with company accreditation standards, including annual training, CEUs, and performance reviews
  • Utilize de-escalation and crisis intervention skills when needed
  • Delegate certain duties to non-licensed staff such as BHTs or Resident Techs
  • Learn and utilize the software available (Ritten, Paycom, etc) efficiently to complete job requirements

The LPN requires the successful completion of an accredited Licensed Practical Nurse program along with:

  • Active LPN license in good standing
  • 2 years of general nursing experience, with at least 1 year of previous psychiatric and/or substance experience
  • De-escalation/crisis intervention experience (preferred)
  • IV insertion and blood draw capabilities are, or willingness to learn
  • Must be open-minded, flexible, and willing to take direction
  • Valid driver's license or reliable transportation
  • Valid CPR/First Aid certifications or ability to obtain within 30 days of hire
  • High level of confidentiality
  • Exceptional display of verbal and written communication
  • Ability to successfully pass a background and drug screen
