Shift: PT - can be per diem or part time, must be able to work at least one day a week, swing shift. 11am-11pm

Position Overview
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN)/Phlebotomist to join our team in Bloomington IN. This role plays a vital part in supporting clients' medical needs throughout their recovery journey. The LPN will provide direct patient care, including performing phlebotomy, conducting EKGs, obtaining and monitoring vital signs, and assisting with admissions. Responsibilities also include monitoring health concerns, completing medical documentation, and assisting in facilitating health-related programming, all while maintaining the highest standards of care and compliance.
Key Responsibilities
  • Monitor infection control practices and follow reporting procedures.
  • Conduct urinalysis toxicology screenings and document results.
  • Administer medications and treatments as directed by providers.
  • Triage medical issues and perform focused patient assessments within scope of practice.
  • Document client symptoms, treatments, and health-related concerns in the EMR.
  • Maintain communication with scheduling coordinators and medical providers.
  • Support health education groups (e.g., HIV/Hepatitis, Nutrition) and complete related documentation.
  • Attend scheduled staff meetings and trainings.
  • Maintain accurate logs, shift reports, and other medical records.
  • Report significant questions, concerns, and complaints to supervising staff.
  • Assist in training Behavioral Health Technicians (BHTs) on medical protocols.
  • Maintain regular, prompt attendance and be available for overtime if required.
  • Perform other duties as assigned by senior management.

Requirements
Qualifications/Required Experience
  • Licensed Practical Nurse (LPN) in the state of Indiana.
  • CPR certification required.
  • Experience in a substance abuse, psychiatric, or related healthcare setting preferred.
  • Strong verbal and written communication skills.
  • Ability to work 8-hour shifts
  • Ability to do vitals and phlebotomy

Education & Licensure
  • Licensed Practical Nurse (LPN) credential in Indiana is required.
  • CPR certification required and must be maintained throughout employment.
