Are you looking for something a little different in a healthcare career?A position that only requires patient assessment as the only direct care and Monday through Friday schedule? If so, then you might be the perfect fit for a MDS Coordinator (LPN) position within the organization.

Under the direction of the Director of Nursing (D.O.N.), theResident Assessment Coordinator (RAC) leads an interdisciplinary team using a core set of screening, clinical, and functional status elements which forms the foundation of a comprehensive assessment. RAC utilizes this comprehensive assessment to ensure the plan of care has a holistic approach centered around the resident's wants, needs and goals. The ideal candidate, to become a Resident Assessment Coordinator, is a nurse with excellent documentation skills and superior assessment skills. She/he is detailed-oriented, employs the nursing process and uses critical thinking. The Resident Assessment Coordinator is an administrative nurse that is a leader and an educator in the post-acute care setting.

The details:

  • Coordinate the interdisciplinary assessment process for assigned residents of the facility.
  • Coordinate development of and ensure completion of Comprehensive Care Plan accordance with company, state and federal guidelines.
  • Monitor residents to ensure that the individual resident Care Plans are followed and updated as necessary. Bring any variances noted to the attention the Director of Nursing and Care Plan Team.
