Job description

In the LPN role within General Internal Medicine, patient visits include, but are not limited to: review of patient health history, medication review, immunization administration, ear lavage, focused nutrition assessments, and blood pressure visits. Telehealth consists of collecting pertinent information ahead of a provider visits, researching prescription discrepancies, and/or facilitating prior authorization denials.  The LPN collaborates daily with providers, desk operations specialists, patient appointment service specialists, interpreters, and medical secretaries.

The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.

Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.


One year LPN experience within the last five years preferred. Ability to adapt to unpredictable situations within the clinical and/or research settings. Able to work effectively with multiple members of the interdisciplinary care team. Demonstrates strong, effective and open communication skills to achieve mutual understanding and build trust and rapport with team members. Able to work in a rapidly changing environment while participating in developing innovative and creative solutions to patient care. Excels in creative experimentation and is able to explore new paths, procedures, and /or approaches to models of care teams. Displays flexibility in adapting and coping with accelerated changes within the environment. 

Internal applicants are required to attach their three most recent performance appraisals. 

License and Certification (Must obtain prior to start date): 

Current LPN license by applicable state requirements. 

BLS (Basic Life Support) must be valid for 90 days post start date; acceptable certifications include: American Heart Association (AHA)/ Heartcode BLS, Military Training Network, Red Cross (CPR/AED for Professional Rescuers and Health Care Providers). 

Additional state licensure(s) and/or specialty certification/training as required by the work area.
