Description:

Hilton East Assisted Living Facility is currently seeking Licensed Practical Nurses (LPNโ€™s) to join our caring and compassionate team. Our LPNโ€™s will play a key role in providing quality care to residents while ensuring that prescribed medications are administered safely and accurately. The LPN will be responsible for assessing residents' health, administering medications, and assisting with other nursing duties in accordance with facility policies and state regulations.


Hiring for:

  • LPN Day Time 10am-630pm
  • LPN Eve 2p-1030pm
  • Per Diem LPN


Key Responsibilities:

1. Medication Administration:

  • Safely and accurately administer prescribed medications to residents as per their individualized care plans.
  • Maintain a clear and up-to-date medication administration record (MAR) for each resident.
  • Follow all medication policies and procedures, including documentation, storage, and disposal, to ensure regulatory compliance.
  • Report and document any adverse reactions or side effects related to medications and notify the physician and family as necessary.

2. Resident Care:

  • Monitor residents for changes in health status and report any concerns to the Resident Care Director and the resident's physician.
  • Assist in daily activities, including vital signs monitoring, blood glucose checks, and other health assessments.
  • Provide personal care as needed, such as assistance with bathing, dressing, grooming, and mobility.
  • Promote a safe and supportive environment for residents, ensuring their comfort and well-being.

3. Collaboration and Communication:

  • Collaborate with other healthcare professionals, including physicians, registered nurses (RNs), and certified aides, to develop and implement care plans.
  • Communicate effectively with residents, their families, and other team members regarding changes in health, treatment plans, and progress.
  • Assist with resident admissions, transfers, and discharges as directed.

4. Documentation:

  • Maintain accurate and detailed medical records in accordance with state regulations and facility policies.
  • Document resident interactions, changes in condition, medication administration, and any incidents in a timely and professional manner.

5. Compliance and Quality Assurance:

  • Ensure adherence to all local, state, and federal regulations, including those regarding medication management and resident rights.
  • Participate in facility audits, quality improvement activities, and in-service training sessions as required.
  • Report any concerns regarding resident safety or facility practices to the supervisor.

Requirements:Education
  • Graduate of an accredited LPN program.
  • Current LPN license in good standing with the NYS Board of Nursing.

Experience

  • Prior experience in an assisted living, long-term care, or similar healthcare setting preferred.
  • Experience with medication administration and knowledge of common medications for elderly residents is preferred.

Skills

  • Proficiency in medication management and documentation.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Strong attention to detail and organizational skills.

Certifications/Other Requirements

  • CPR and First Aid certification (current).
  • Ability to pass background checks and meet all health and safety requirements.

Work Conditions

  • Full-time or part-time positions available.
  • Shifts may include evenings, nights, weekends, and holidays, depending on the facilityโ€™s needs.
  • Physical demands include the ability to stand, walk, bend, and lift residents or equipment as necessary.

Compensation

  • Competitive salary based on experience, starting at $29/hr.

Compliance Requirements

  • Must have New York State Nursing License .
  • Must have Valid NYS Drivers license
  • Must have documentation of MMRโ€™s (Measles, Mumps and Rubella, showing an immune result.)
  • Must have current Physical (within 30 days of employment) .
  • Must have current PPD test with a negative result.
