We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Rivers Edge Nursing & Rehab in Muscoda, Wisconsin. As a Licensed Practical Nurse (LPN), you will play a vital role in providing high-quality care to our residents while working a dynamic and rewarding evening shift.

About the Role:

As an Evening Shift LPN, you will be responsible for:

  • Providing direct patient care to residents in our long-term care facility, including administering medications, monitoring vital signs, and assisting with daily living activities

  • Working collaboratively with our interdisciplinary team to develop and implement individualized care plans

  • Maintaining accurate and detailed records of patient care and medication administration

  • Providing emotional support and companionship to our residents and their families

Responsibilities:

  • Perform routine vital signs, such as temperature, blood pressure, and pulse

  • Administer medications and treatments as prescribed by the physician

  • Assist residents with daily living activities, such as bathing, dressing, and grooming

  • Participate in care conferences and develop individualized care plans

  • Maintain accurate and detailed records of patient care and medication administration

  • Provide emotional support and companionship to residents and their families

Requirements:

  • Current LPN license in the state of Wisconsin

  • High school diploma or equivalent required; Associate's degree in nursing or related field preferred

  • Previous experience in a long-term care setting preferred
