2nd Shift | 2:00 PM – 11:00 PM Pay: $34.81 Start Date: Approximately 2 weeks from offer acceptance. Assignment Type: Temporary assignment with the opportunity for extension or conversion to a permanent position based on business needs and your performance in the role.

Position Overview:

The 171466 Licensed Practical Nurse (LPN) is responsible for providing high-quality nursing care to residents while working under the direction of the Registered Nurse (RN). This role includes medication administration, resident assessments, documentation, infection prevention, collaboration with the healthcare team, and ensuring residents receive safe, compassionate, and effective care.

Primary Responsibilities:

Administer medications and treatments accurately and on time as prescribed by physicians. Order, receive, and verify medications according to established procedures. Count narcotics and maintain accurate controlled substance records. Verify medication deliveries and notify the pharmacy of any discrepancies. Participate in feeding residents and communicate concerns to the RN. Provide specialized nursing care to residents on specialty units, including Alzheimer's and memory care.

Conduct resident rounds and verify resident census to ensure safety and well-being. Observe resident conditions and promptly report changes to the appropriate clinical staff. Accurately document nursing care, medications, treatments, and assessments within the electronic medical record (EMR). Assist with resident admissions, transfers, and discharge documentation. Collaborate with the RN to develop, implement, and update resident care plans. Monitor resident appearance, hygiene, and overall comfort.

Observe resident care during meal times and throughout the facility. Receive shift reports and provide comprehensive handoff reports to incoming nursing staff. Monitor residents for injuries and immediately notify the RN Supervisor of any concerns. Communicate resident participation status for activities and therapies. Follow all Infection Control guidelines and Universal Precautions. Utilize required personal protective equipment (PPE), including gloves, masks, gowns, goggles, and face shields.

Assist with daily resident health assessments and communicate findings to the healthcare team. Help orient and mentor new LPNs and Certified Nursing Assistants (CNAs). Monitor and provide direction regarding CNA performance related to resident care. Maintain adequate nursing supplies and communicate inventory needs to nursing leadership. Evaluate resident care plans, nursing procedures, and recommend process improvements. Attend required meetings, conferences, competency assessments, and continuing education programs.

Support Quality Assurance Performance Improvement (QAPI) initiatives and participate in Performance Improvement Projects (PIPs). Follow workplace safety policies and immediately report injuries, incidents, or safety hazards. Perform additional duties as assigned.

Required Qualifications:

Current Licensed Practical Nurse (LPN) license in good standing. Current Cardiopulmonary Resuscitation (CPR) certification or ability to obtain and maintain certification. Ability to administer medications safely and accurately. Experience documenting care using electronic medical records (EMR). Knowledge of medication administration procedures and controlled substance documentation. Understanding of infection prevention and control practices. Strong assessment, observation, and documentation skills. Excellent verbal and written communication skills. Ability to work collaboratively with RNs, CNAs, physicians, and interdisciplinary healthcare teams. Strong organizational and time management skills. Ability to maintain resident confidentiality and professionalism.

Preferred Qualifications:

Experience working in long-term care, skilled nursing, rehabilitation, or memory care settings. Experience caring for residents with Alzheimer's disease or other dementia-related conditions. Experience supervising or mentoring Certified Nursing Assistants. What You Should Know Before Applying: This is a 2nd shift position working from 2:00 PM to 11:00 PM. The expected start date is approximately 2 weeks after accepting an offer.
