Black Hills Special Services Cooperative is seeking an LPN–Case Nurse to support individuals with developmental disabilities by coordinating their medical care and helping them achieve personal health goals and needs. You'll manage caseloads, monitor medications and treatments, and collaborate with direct support staff and healthcare providers to ensure timely, person-centered care. This position is ideal for a licensed LPN who is organized, communicative, and passionate about making a difference in the lives of others through compassionate nursing and thoughtful case management.

Black Hills Special Services Cooperative (BHSSC) is a public entity that is an educational cooperative under the State of South Dakota. BHSSC is comprised of five divisions dedicated to aiding school districts, individuals, and the communities.

Job Description-

  • Maintain a caseload of individuals receiving services and support their ongoing health, wellness, and medical needs. 
  • Coordinate medical appointments, referrals, preventive care, follow-up services, and healthcare recommendations.
  • Monitor health conditions and communicate changes in health status to ensure timely intervention and support.
  • Assist with medication management, including monitoring medication systems, refills, documentation, and medication-related concerns.
  • Complete health assessments, healthcare reports, care documentation, and nursing-related records in accordance with regulatory requirements.
  • Collaborate with individuals served, families, guardians, healthcare providers, pharmacists, and support teams to ensure coordinated care.
  • Provide health-related training, coaching, and support to Direct Support Professionals and other team members.
  • Promote person-centered healthcare, healthy lifestyles, independence, and informed healthcare decision-making.
  • Participate in quality improvement, risk management, incident review, and health and safety initiatives.
  • Support nursing on-call responsibilities and assist during urgent or emergency situations as needed.
  • Other duties as assigned
 

Job Qualifications-

  • Current, unrestricted South Dakota Licensed Practical Nurse (LPN) license. 
  • Valid South Dakota driver's license and an insurable driving record. 
  • Ability to pass required background checks, drug screening, and agency training requirements. 
  • Strong clinical assessment, communication, organization, and problem-solving skills. 
  • Ability to build positive relationships with individuals served, families, healthcare providers, and team members.
  • Experience working with people with disabilities, case management, community health, long-term care, home health, or related nursing settings preferred.
  • Proficiency with electronic health records, medication records, Microsoft Office, and other technology systems.
  • Ability to manage multiple priorities, travel between service locations, and respond effectively to changing situations
