A licensed practical nurse (LPN) care manager coordinates and oversees a plan of care for patients living at home. In this career, you assess the environment, functional and psychosocial status, and financial situation of a patient, consult their physician, and develop a program or action plan to empower them as they go through care. You clarify treatment plans, monitor medication regimens, and address other urgent issues. LPN care managers are caregivers that also document care and improvement and provide support to family members. You collaborate with other professionals and do whatever is needed to assist with the care of your patient.
