Responsibilities
  • Medication & Treatments: Prepare and administer medications (including injections and prescribed treatments) and transcribe physician orders accurately.

  • Clinical Monitoring: Monitor vital signs, track fluid intake/output, and observe patients for progress or medication side effects.

  • Documentation: Maintain accurate patient charts, medical histories, and daily observations.

  • Direct Care: Assist patients with daily activities, including wound care, dressing, and hygiene.

  • Support & Communication: Provide emotional support to patients and communicate effectively with family members regarding care instructions.

  • Collaboration: Perform additional duties as assigned by the Director of Nursing to ensure high-quality care.

Qualifications
  • License: Must hold an active and unrestricted Licensed Practical Nurse (LPN) license in the state of Illinois.
