Company Description
At Wellness Pointe, everyone will receive an exceptional quality of care. The center has expanded into a multifaceted clinic that offers obstetrics, pediatrics, general medicine, family planning, and dentistry. Some of the other services offered by our well qualified staff include education, intervention, and management of the many social issues that impact our community. Everyone is treated in complete confidence. Medicaid, Medicare, and most private health insurance is accepted.
Job Description
The Intervention Specialists will work in the PADRE program which provides community support for mothers and fathers with substance use disorders; or, those who are at risk of developing substance use disorders and assist the family to thrive. The goal of this interaction is to provide a planned and coordinated educational approach to decrease the effects of substance use within the family and to increase access to community resources and evidence informed education. (PADRE- Parenting Awareness Drug Risk Education Services)
Essential Functions:
Position will represent the program at relevant meetings throughout the community, and be familiar with community resources in order to conduct referrals and meet the needs of the targeted groups
Will coordinate and work closely with Practice Coordinator, relevant social service and behavioral health programs of Wellness Pointe. Will also include reaching out to other relevant programs, to include faith-based programs, within the region to assist in identifying appropriate PADRE candidates
Will work closely with the patients in an effort to keep them engaged in the medical system during pregnancy and facilitate referral to substance abuse treatment services as warranted
Position will communicate with staff any concerns or issues needing immediate attention
Assists senior staff in developing new program components, and other related duties
Carries out other duties as assigned
Qualifications
Minimum Requirements:

  • Requires minimum of Associate's Degree in a behavioral health field of study; or 2 years case management experience in a behavioral health field; or Qualified Credentialed Counselor (QCC) prefer Licensed Chemical Dependency Counselor (LCDC);or LCDC-Intern; or LPC-Associate; or Certified Criminal Justice Addiction Professionals-Applicant (CCJP-A)
  • Knowledge of available prenatal care systems and postpartum services
  • Ability to conduct screenings, assessments, service plans, and follow-up with identified patients
  • Requires a valid Texas Driver's License, appropriate driving record and auto liability insurance that meets the minimum required by State of Texas statute
  • Mental and physical health sufficient to meet the demands of the position

  • Must be able to communicate effectively in large group settings and demonstrate independent judgment
  • Must document daily activities in CMBHS System
  • Ability to understand the overall goal of the program and implement activities that meet objectives
  • Must be able to keep up-to-date and factual documentation

  • The ability to work independently and effectively under pressure.
  • Proficient in MS Office Suite

Physical Requirements
Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours.
The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.
The occupant may, at times, lift and/or move objects up to 30 pounds. The noise level in the work environment is usually moderate.
