Job description

Liquid Personnel is recruiting for an experienced Locum Occupational Therapist to join its client inย Wiltshire Council's Reablement Team, supporting individuals following hospital discharge to regain independence and achieve positive outcomes within the community.

This is a fantastic opportunity for an experienced OT withย reablement, community, hospital discharge or Adult Social Care experience, working across home, hospital and community settings.

What will your responsibilities be?ย 

  • Complete Occupational Therapy and functional assessments for individuals following hospital discharge
  • Develop tailoredย reablement plansย focused on restoring independence and reducing ongoing reliance on care and support
  • Work across home, hospital and community settings to assess functional needs and identify practical solutions
  • Provide specialist advice aroundย equipment, safe practice, mobility, transfers and independence
  • Work collaboratively with Social Workers, health professionals, care providers and the wider multidisciplinary team
  • Support decision-making around care, risk, rehabilitation and ongoing support needs
  • Monitor progress and review interventions to ensure reablement goals remain appropriate and outcomes-focused
  • Maintain accurate and timely clinical records and manage a varied caseload effectively
  • Promote person-centred, strengths-based practice, enabling individuals to maximise their independence following hospital discharge

Contract:ย Locum - 3 months initially, with potential for extension
Hours:ย 37 hours per week
Working Pattern: Hybrid - approximately 50% office-based administration, with remaining time split between home working and community visits
Please note: Wiltshire Council does not cover accommodation or travel expenses from home for candidates based outside Wiltshire.ย 

Qualifications & Experience:ย 

  • Qualifiedย Occupational Therapist
  • Currentย HCPC registration
  • Minimumย 2 years' post-qualification experience
  • Experience in reablement, community OT, hospital discharge or Adult Social Care is desirable
  • Strong assessment, clinical reasoning and risk management skills
  • Fullย UK driving licence and ability to travel to community visits
