Job description

Position Summary: Provide a range of psychological assessment, diagnostic, therapeutic, and/or case management services for individuals, couples, groups, and/or families.


Location: This position is required to be in person at any Infinity Health location.


Licensure: This position requires a valid LISW, LMHC, or LMFT licensure in the State of Iowa.

Essential Duties and Responsibilities:

  1. Practices exceptional customer service to all
  2. Supports the mission, vision, and values of Infinity Health
  3. Provide psychosocial assessments of residents and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation
  4. Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, and family and/or group therapies
  5. Provide therapeutic crisis intervention and emergency services as required
  6. Assist with integrating each individual into their home/community
  7. Participate in staffing to present case histories; confer with internal and external members of therapy team to develop comprehensive evaluations or plan pertinent therapy programs to provide maximum benefits for each patient
  8. Prepare and maintain a case record for each patient to describe the nature of social problems and services suggested or provided
  9. Provide education to patients and families around issues related to adaptation to the patient’s diagnosis, illness, treatment and/or life situation.
  10. Maintain working knowledge of and liaison with community agencies and resources
  11. Provide consultative services to health care team members within scope of care definitions as needed
  12. Participate in Quality Improvement activities, including data collection, tracking, and analysis
  13. Attend regularly scheduled meetings, training, and workgroup/committee activities to enhance team performance
  14. This position may be allowed to work remotely
  1. Assist with the implementation and sustaining of patient centered medical home status as needed
  2. Complete other duties as assigned

Qualifications:

  • Master’s Degree in Behavior Science or other related discipline
  • Valid LISW, LMHC, or LMFT Iowa License
  • 3 years of experience in related field
  • All employees are required to comply with Infinity Health’s vaccination policy.
