Job Title: Behavioral Health Care Manager (Field-Based) Payrate: $40-$43 an hr Location: On-site/In-field – Northwest Ohio (Lucas, Fulton, Wood, Ottawa Counties) Job Type: W2 Contract (Temporary) | Full-Time (40 hrs/week) Contract Length: ~52 weeks (estimated June 2, 2026 – June 1, 2027 ) Job Summary We’re hiring a Behavioral Health Care Manager to provide care management services for members, including assessment, care planning, coordination of services, and ongoing follow-up. This is a field-based role supporting members across multiple counties in Northwest Ohio. You must be comfortable working independently, communicating clearly, and adapting to changing priorities while partnering with a larger care team.

Key Responsibilities Provide care management services in accordance with best practices Assess member needs and develop individualized care plans Coordinate with providers, care teams, and family/support systems to support treatment goals Monitor progress and update care plans as needed Educate members and families on available resources and support services Document interventions and communicate updates to appropriate stakeholders Required Qualifications Education (one of the following): Master’s degree in Behavioral Health or Social Work, OR Degree from an Accredited School of Nursing Active Ohio license (required – OH is not a compact state): LCSW, LMSW, LMFT, LMHC, or LPC Behavioral Health experience is required 2–4 years of related experience, such as: Behavioral health care management / case management Nursing (case management), home health, behavioral health setting Reliable vehicle and ability to travel weekly within the assigned territory Ability to work independently in a field-based environment with strong time management Preferred Qualifications 2–4 years of Behavioral Health–specific experience Additional Qualities We’re Looking For Works well independently and is personable Strong communication (phone, documentation, and stakeholder updates) Critical thinker who can problem-solve and prioritize Flexible, adaptable, open to change, and able to collaborate effectively on a team Disqualifiers (Please Note) Inability to work independently Difficulty managing change/fast-moving priorities Candidates seeking long-term permanent roles only (this is a contract position) Benefits (W2) Benefits available to full-time employees after 90 days 401(k) with company match available after 1 year of service How to Apply Click Apply on ZipRecruiter.
