Position Overview

The Residential RN provides clinical oversight and guidance to staff supporting individuals in residential settings. Working under the supervision of the Lead RN, this role focuses on care coordination, staff training, regulatory compliance, and ensuring the highest standards of health and safety.

You will collaborate closely with interdisciplinary team members to support holistic, person-centered care.

Key Responsibilities

Clinical Oversight & Collaboration

  • Provide clinical guidance to residential staff supporting individuals with ID/DD
  • Collaborate with the Lead RN, Clinical Director, PBS clinicians, and interdisciplinary team
  • Participate in team meetings to review plan of care and health outcomes

Health & Medication Management

  • Serve as a Medication Administration Program (MAP) consultant in accordance with DPH policies
  • Conduct physical assessments and respond to changes in individualsโ€™ health status
  • Support safe hospital discharges and follow-up care

Training & Consulting

  • Provide MAP and health-related training to staff under the direction of the Lead RN
  • Serve as a resource for residential staff regarding incident follow-up, medical needs, and safe hospital discharges.

Compliance & Quality Assurance

  • Perform MAP audits and ensure compliance with DDS and DPH regulations
  • Adhere to standards set by DDS, DPPC (mandated reporting), and OQE indicators
  • Monitor infection control practices and safety procedures

Documentation & Reporting

  • Maintain accurate and timely medical records and documentation
  • Report and document medication errors, incidents, and health changes

Program Participation

  • Attend ISP meetings, MAP meetings, and medical appointments as needed
  • Support continuous quality improvement and best practices in care delivery
Required Qualifications
  • Active Registered Nurse (RN) license in the state of Massachusetts
  • Minimum of 2 years of RN experience (residential or community-based experience preferred)
  • Strong communication, organizational, and documentation skills
  • Ability to work effectively within a multidisciplinary team
  • Compassionate, patient-centered approach to care
  • Self-motivated and able to work independently
  • Must have reliable transportation and be willing to travel between residential sites throughout the workday within the greater Springfield area.
Preferred Qualifications
  • Experience working with individuals with intellectual and developmental disabilities
  • Strong knowledge of the Medication Administration Program (MAP)
  • Experience conducting audits and ensuring regulatory compliance
  • Experience training or presenting to groups
Supervision

This position reports directly to the Lead RN, who provides ongoing guidance, mentorship, and support.
