Parkview Physicians Group is currently seeking experienced Board Eligible / Board Certified Family Medicine or Internal Medicine physicians to join our Transitional Care Clinic team in Fort Wayne, Indiana.

Specifics of The Role

  • Part-Time role – 20 hours per week to include rounds Monday - Friday
  • Involves travel by conducting daily rounding on Parkview patients in high performing network skilled nursing facilities
  • Nurse practitioners cover call; physician would need to be available as a backup by phone

The Team

  • Group consists of 2 physicians & 5 advanced practice providers that provide quality continuation of care to patients that are discharged from the hospital
  • Comprehensive care team and clinical support model comprised of medical assistants and registered nurses
