This Home Health Occupational Therapist position is based in the welcoming community of Danbury, Connecticut, a city that blends classic New England charm with modern conveniences. Your typical day will include traveling to patient homes, performing comprehensive evaluations, collaborating with the broader home health team, and delivering one-on-one, functionally focused treatments tailored to each patient’s living environment. Core responsibilities include completing detailed functional assessments, including home safety evaluations and analysis of activities of daily living and instrumental activities of daily living. You will develop and implement individualized plans of care, focusing on maximizing independence in self-care, mobility within the home, household tasks, and community participation. You will recommend and train patients and caregivers in the use of adaptive equipment and durable medical equipment, instruct on energy conservation and joint protection strategies, and provide education to reduce fall risk and promote safe movement. You will work closely with nursing, physical therapy, social work, and other disciplines to coordinate services, update care plans, and monitor patient progress. Typical caseloads are managed to allow for meaningful one-on-one time in the home, with visit lengths designed to support thorough assessment, intervention, and patient and caregiver education. Patient populations commonly include older adults recovering from joint replacements or fractures, individuals managing cardiac or pulmonary conditions, patients after hospital or skilled nursing facility discharge, and those living with neurologic conditions such as stroke or Parkinson’s disease.
