Pay: $71,000-$79,000/year, based on experience

This position is paid on a per-point basis. The compensation reflected on this posting is an estimate of annual compensation.

Coverage Area: Warren & Claiborne Counties, MS

Schedule: M-F 8a-4:30p | on-call rotation

As a Home Health Registered Nurse (RN) Case Manager, you will lead and manage patient care from admission through discharge—bringing clinical expertise, compassion, and coordination to every interaction. You’ll serve as the central point of contact for patients, families, physicians, and the interdisciplinary care team, ensuring care plans are executed safely, effectively, and with a focus on quality outcomes. In this role, you’ll combine hands-on nursing care with care coordination, education, and clinical oversight to support each patient’s unique journey.

RN Case Manager Qualifications

  • Graduate from an approved school of professional nursing
  • One (1) year of experience as an RN
  • Licensed to practice as a registered nurse in Mississippi
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
