Individual Home Care Provider, Inc. located in Centereach, NY, is a New York State Licensed Home Care Agency. We service patients in Suffolk, Nassau, and Queens. We employ home health aides with either a PCA or HHA certificate. There are live-in or live-out cases available for variable days, including 7 days.

Job Description

We are looking for an RN  that will perform initial intake assessments, supervisory visits and follow-up visits to home care patients. This RN will primarily be asked to see patients in the Queens and Western Nassau Regions.

Qualifications

NYS RN license, current PPD/Chest X-ray, current physical, MMR record, and liability insurance.
