We are currently seeking an Assessment Specialist to join our Specialized Recovery Support Services program. As an Assessment Specialist, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
  • Maintains and builds knowledge of the State and program requirements.
  • Maintains and builds on knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
  • Completes in-home visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
  • Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy, including standards of content and timeliness.
  • Provides assistance with community resource referrals as appropriate.
  • Adhere to the CareStar Rule in performance of job responsibilities.
  • Understand and comply with CareStar Policies and Procedures.
  • Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Minimum Qualifications
  • Possesses a Bachelor's Degree in social work, counseling, psychology, or a related field; or Registered Nurse (RN), or Licensed Social Worker (LSW).
  • Possesses a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years' post degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.
  • Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.
  • Experience working with individuals with chronic conditions or severe and persistent mental illness
  • Strong knowledge of Medicaid, behavioral health systems, and local community resources.
