Position Purpose:

All KHI employees are expected to perform their respective tasks and duties in such a way that supports KHI’s vision to be among the kindest, highest quality health systems in the country.

The incumbent shall be responsible for providing a full range of Physical Therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards. With each patient encounter, the incumbent shall use AIDET, perform hand hygiene, assists with positioning patient for comfort and pain management, and performs environment check prior to leaving patient’s room and/or maintains a clean and organized environment.

Key Responsibilities

  • Conduct a Physical Therapy evaluation or screen.
  • Develop an individualized Plan of Care with functional goals.
  • Provide appropriate therapeutic interventions.

· Evaluate patient response to treatment and modify plan as necessary

  • Document the course of patient care including progress made and the continuing need for treatment.
  • Provide patient and/or family education to help achieve goals.
  • Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others.
  • Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, and Medicare.
  • Assists with orientation of new staff as requested.
  • Meets KHI requirements for skills competency and safety.
  • Participate in continuing education/professional development activities.
  • Assists with the Physical Therapy and Physical Therapy Assistant student program.
  • Participate in staff meetings, client relation and marketing activities.

· Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy, and Medicare regulatory requirements.

· Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation.

· Individual productivity meets department standards.

· Patient satisfaction will meet or exceed department standards.

Qualifications

Education: Graduate of an APTA approved four year bachelor’s degree program, or foreign equivalent.

Licensure: Current PT License through the AZ Board of Physical Therapy

Certification: BLS through the American Heart Association or American Red Cross

Experience: Recent experience working with an adult and geriatric population
