Job description

Sample Supports is actively seeking an energetic and professional Host Home Provider in the Larimer/Weld County areas to offer a home to a female individual in her 30s with intellectual disabilities, as well as medical and mental health needs. Host Home Providers offer an in-home placement to adults with disabilities of varying needs, allowing each person to live in their own communities with the support they need to stay as independent as possible.


This position will serve the individual with her behavioral needs, mental health needs, medical needs, and daily support needs. The individual needs support in general activities of daily living, social and emotional regulation, plus encouragement to participate in needed appointments and services. The ideal candidate has experience with complex medical needs and monitoring for signs of infections or behavioral escalation.

Host Home Providers are contractors of Sample Supports and receive competitive payments for the difficulty of care required for the person.


Sample Supports provides comprehensive general training as well as specific training tailored to the individual in care. Our residential team is on-call 24/7 and is there to support you if any needs come up during the placement. You become a part of our integrated care team, and we aim to create placements that fully integrate into both your home and the greater community.


Sample Supports will provide:

  • General training on caregiving and the needs of individuals with disabilities
  • Trauma-informed and social emotional care responses
  • First-aid, CPR, and general safety trainings for all members of the household
  • Person-centered training based on the person in care
  • Integrated and adaptive behavior support plans
  • Crisis intervention and emergency response planning
  • 24/7 on-call support
  • Support team of expert clinicians to provide in-home and community-based training as needed
  • Up to 30 hours/week of supplemental programming for the individual if needed
  • Trained respite providers and proactive respite scheduling support


Provider duties and responsibilities:

  • Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment
  • Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed
  • Community activities based on the needs of the individual - can create a unique activity plan together for their respite stay
  • Transportation services as needed - may include transportation to/from scheduled activities and appointments
  • Complete documentation for services provided
  • Medication administration and support as needed

Requirements for the contract:

  • Must be at least 21 years old
  • Must have a GED or diploma
  • Must have dependable and reliable transportation with the ability to safely transport
  • Must have a valid driver's license
  • Must be able to pass background check
  • Must have appropriate living environment with at least one available bedroom
  • Preferred knowledge and understanding of developmental disabilities, mental health, and trauma
  • Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained
  • Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at-risk individual (M/A/N/E). Each position is considered a mandatory reporter


Preferred:

  • Non smoking and healthy lifestyle
  • No young children and minimal number of pets, if any
  • Maintain open communication with individuals guardians
  • Fiscally responsible
  • CNA, LPN, or RN


Compensation varies depending on the level of support the individual needs. This placement will be compensated based on the current difficulty of care rates. Supplemental room and board payments may also be provided at up to $797/month.
