About the Unit: Moore Place is part of DeMay Living Center, housed on the 3rd floor of Newark- Wayne Community Hospital- 1200 Driving Park Newark, NY 14513. Moore Place Neurobehavioral Rehabilitation Program, is a 20 bed (private rooms) floor specializing in treating residents who have diagnoses of Alzheimer's and/or dementia that have unpredictable aggressive behaviors. Residents that have Alzheimer's, Parkinson's, and others that have verbal and/or physical aggression, who are unable to have their needs met on a regular dementia floor come to Moore Place for stabilization and discharge planning. Our program is designed to be a year or less, depending on the resident's condition. Each case is unique and often complex. We focus on developing behavioral management strategies based on each individual's needs. The program addresses: Alzheimer's and/ or Dementia diagnoses with behavioral disturbances in a secured and structured environment, medication management, developing daily successful routines, and discharge planning. Residents come from the community, hospitals, nursing homes, and assisted living. The expectation is that once stabilized, the resident discharges/returns to their previous setting. It's crucial to focus on discharge planning and setting, as residents can't stay on Moore Place for Long Term Care. Facilities must sign an agreement that they will accept resident back once stable. Barriers to Admission: Due to the behavioral nature of our population, safety is crucial. We can't accept people requiring oxygen/tubing, intravenous therapy, feeding tubes, any lines/ports/drains, insulin pumps, catheters, or Nephrostomy tubes. Residents work with trained medical professionals to achieve their maximum health and functional ability, so that they are able to be successful and manage a daily routine following discharge. Each resident requires stabilization of their behaviors in order to live in the least restrictive environment.
