Job Descriptionย 

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The LPN will provide direct nursing care to residents, support individualized care planning, and ensure high-quality clinical services in accordance with state and federal regulations. This role is ideal for a nurse who is patient-focused, detail-oriented, and passionate about making a positive impact in the lives of others.ย 

Responsibilitiesย 

  • Assess residentsโ€™ physical and emotional needs and report changes in conditionย 

  • Provide direct nursing careย in accordance withย established care plans and physician ordersย 

  • Assistย in developing, implementing, and updating individualized care plansย 

  • Administer medications and treatments as prescribedย 

  • Supervise Certified Nursing Assistants (CNAs) and support staff to ensure safe and effective care deliveryย 

  • Maintainย accurateย andย timelyย documentation in compliance with regulatory standardsย 

  • Ensure adherence to state and federal rules and regulations governing nursing practice within the facilityย 

  • Promote a safe, respectful, and compassionate care environmentย 

Qualificationsย 

  • At least two years of experience in Skilled Nursing/LTC/Rehabย 

  • Current Licensed Practical Nurse (LPN) license in Oregon (OR)ย 

  • BLSย requiredย
