Join our Aegis Family as a Home Health and Hospice Licensed Practical Nurse (LPN) where you will play a crucial role ensuring our patients receive exceptional care. Our LPN's are responsible for verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. This includes providing essential nursing treatments, promptly addressing patient pain, assisting with activities of daily living, and educating both patients and caregivers on self-care techniques. They also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions. Desired Skills and Qualifications

  • Current Licensed Practical Nursing License in state of practice
  • 1+ years of Home Health & Hospice Experience preferred
  • Ability to work independently, be self-directed, and flexible.
  • Ability to use a full range of motion and ability to lift 35lbs
  • Effective verbal & written communication & time management skills
  • Valid Driver's License (clean record) & reliable transportation.
  • Proof of current auto insurance & registration
