Job description

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, development/intellectual disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a qualified candidate who is interested in joining our ACT Alliance team in Georgetown, DE.
The Associate Mental Health Specialist, as part of the ACT multidisciplinary team, provides rehabilitation and support functions and assist in treatment, provide substance abuse services, education, support and consultation to families; and provide crisis intervention under clinical supervision.
Some of the responsibilities would include the following:
  • Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
  • Provide service coordination (case management) for assigned group of participants including coordinating and monitoring the activities of the individual's treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising the overall treatment goals and plans in collaboration with the participant and the Team.
  • Provide on-call crisis intervention covering nighttime hours.
  • Provides symptom education to enable participants to identify their mental illness symptoms.
  • Coordinate with state vocational rehabilitation and other employment services.
  • Provide treatment, rehabilitation and support services with some interventions directed or performed by staff with specialty training and skills (e.g. vocational specialist).

Qualifications:
  • Bachelor's degree in social work, psychology, rehabilitation or mental health counseling, or another related human services field.
  • Knowledge of mental health and substance abuse systems and supports.
  • Ability to work independently.
  • Good communication skills both oral and written.
  • Possess a valid driver's license with an acceptable driving record.

Scope:
  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, reach, stack, stoop, and bend which may include direct intervention with participants.
  • Local travel required, including transporting program participants.
  • Requires carrying a cell phone and being available to respond to emergency situations.
  • May be required to report to work during emergencies, including inclement weather.
  • May require crisis intervention in emergency situations.
