Job description

Building Location:Building F - Duluth Clinic - 3rd StreetDepartment:2901070 NEPHROLOGY - EH DCJob Description:Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention.Education Qualifications:

This position will assist our patients by utilizing a strong team-based approach to care, with a high integration of APP and MDs for the clinic, and inpatient and dialysis units for all 3 phases of nephrology care.

Desired skill set: Experience in complex chronic disease management

Typical Shift: up to 0.4 FTE per pay period

5% clinic work

5% hospital work

90% dialysis rounding

Ideal candidate does not need to live in the Duluth area; candidate could live in areas near Virginia, MN or Grand Rapids, MN as there is travel involved with this position

Schedule: Variable start and end times due to travel

Estimated Patients Per Day: 12 (office), 12-15 (dialysis). Adult patients only (18+)

Other responsibilities: Assist Physicians with non-encounter work
