This position is a contracted 1099 position, working with individuals with disabilities through the Indiana Medicaid Waiver. Waiver Services Supervisor
POSITION OBJECTIVE
The primary responsibility of the Behavior Consultant will be to help clients identify and address problem behaviors through the creation and implementation of a Functional Assessment and Behavior Support Plan. The goal of the Behavior Consultant is to decrease maladaptive behaviors by working with the client and his/her team to teach appropriate replacement behaviors. Behavior Consultant will work with individuals to teach positive replacement behaviors as a means to support and sustain ways they can live actively and independently within their communities. The Behavior Consultant will report directly to the Program Manager who may make changes in job duties from time to time.
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Completion of behavioral consultation with clients, family members and/or guardians and any other members of the client's treatment team (such as: physicians, therapists, etc.).
  • Completion of Functional Assessments (FA), Behavior Support Plans (BSP), and/or Human Rights Committee(HRC) Forms.
  • Attendance and participation in Team meetings.
  • Attending any meetings/trainings scheduled by Program Manager, HSPP or Sky Point.
  • Obtain all needed signatures for required paperwork including intake/annual documents.
  • Responsible for bringing paperwork to the meeting for the purpose of filing copies in the main file.
  • Responsible for making sure clients' files are current and up-to-date both in the main file and the home file.
  • Completion of training on the BSPs and behavior documentation procedures with clients, family members and other providers as needed and filing completed trainings in the main file.
  • Completion of daily progress notes within a 48-hour time frame.
  • Completion of monthly report for each client no later than the 1st day of the month at 8am for the previous month.
  • Submit/upload monthly reports no later than the 1st day of each calendar month.
  • Ensure all billing/payroll units and hours are accurate prior to submission for billing/payroll.
  • Consistent home visitation and follow up with service providers and family.
  • Exhibit effective correspondence and communication with all providers, family members, staff members, clients and all other treatment team members as applicable. Be sure to return phone calls, text messages, and emails within a 24-hour time frame.
  • Maintain a commitment to professional development in compliance with continuing education requirements as prescribed by Medicaid Waiver Program which dictates maintaining at least 10 hours of CEUs yearly.
  • Carry and maintain current professional liability insurance.
  • Adhere to professional and ethical interaction policies and procedures.
  • Responding to crisis calls as soon as possible.
  • Experience with Medicaid Waiver is helpful though we will train motivated individuals who have experience working with persons with developmental disabilities
  • Position requires travel to consumer's homes, must be willing to use personal vehicle and provide proof of current automobile insurance and registration
  • Position consists of growing a caseload as referrals are received

QUALIFICATIONS AND EDUCATION
  • Possess a Master's degree in Psychology, Sociology, Social Work, Special Education, Rehabilitation Counseling, Mental Health Counseling, Marriage and Family Therapy, or Applied Behavior Analyst.
  • Must possess a commitment to the belief that all people have the right to dignity, respect, opportunity and full community inclusion.
  • Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization
  • A report of "no findings" when criminal history checks are completed.
  • Prior experience with persons with developmental disabilities required.
  • Must possess a valid driver's license, automobile registration and insurance.
