Circadia Health Home Health Nurse Practitioner

Circadia Health is a growth-stage healthcare AI company on a mission to prevent avoidable hospitalizations and transform senior-care operations. Our Circadia Intelligence Platform combines:

The Role Transform home-based care with AI. As Circadia's home health NP, you'll see patients first in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions—reducing preventable hospitalizations. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting.

  • Direct patient care: Conduct initial and follow-up in-home assessments on flagged patients; initiate/adjust plans of care; complete consent; coordinate with referring and primary care providers.
  • Tech → bedside integration: Train and coach caregivers and home health aides at the point of care, standardize use of Circadia into home health clinical workflows
  • Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.
  • Close the loop: Complete documentation/charts within the same day
  • Home-based visit management: Efficiently complete 6–8 in-home visits per day across designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts.
  • Quality & product feedback: Surface field insights to improve workflows and tech integration in a home health setting
  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
  • Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.

How Success is Measured (KPIs)

  • Visits: Initially targeting around 6 days per month, with flexibility to increase over time (10+ days)
  • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
  • Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time.

Must-Have Qualifications

  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
  • Eligible for Medicare participation; company bills on your behalf.
  • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
  • Valid CA driver's license; reliable transportation for in-home visits across the SoCal/OC area.
  • Comfortable conducting face-to-face visits in a home environment, including with patients requiring complex care.
  • Thrives in a dynamic, evolving startup environment.
  • English required.

Nice-to-Haves

  • Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
  • Spanish preferred.
