Lifeworks is hiring a full time Assistant Case Manager to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve .
Essential Duties:
  • Provide individual and small group instruction in self-care, activities of daily living, communication, social and motor skills.
  • Provide direct care for ADLs and group activities; Lift and transfer individuals as appropriate to their needs.
  • Participate in scheduled supervision sessions with Assistant Director.
  • Communicate respectfully and pro-actively with families, residential staff and all members of participants’ teams.
  • Participate in program-wide staff meetings and case review meetings as scheduled.
  • Participate in team meetings regarding general program development, including curriculum development and schedules.
  • Attend in-service trainings as required.
  • Comply with all applicable State and Federal regulations, agency policy and procedures, and personnel practices.
  • Protect the health and safety of all participants and staff.
  • Excellent interpersonal skills.
  • Ability to work in a team environment.
  • Perform other duties as requested by supervisor(s).
​Requirements:
  • Experience working with Individuals with ID/DD
  • A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
  • Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
  • Valid Driver's License required
  • Valid MAP certification is preferred
  • Excellent verbal and written communication skills
