This position offers flexibility as a fully remote role, allowing you to manage your schedule effectively while serving our clients’ needs. Typical working hours are from Monday to Friday, 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client necessity and availability. As part of our commitment to adaptable service, you may also be called upon for on-call duties to address urgent client situations. • Perform detailed assessments of seniors' psychosocial needs, including their emotional, mental, and social wellbeing.
• Collaborate closely with clients and their families to develop personalized care plans that reflect their unique goals, preferences, and challenges.
• Offer empathetic counseling and emotional support to clients navigating significant life transitions, such as moving to assisted living or dealing with health issues.
• Advocate effectively for clients' rights, ensuring they access needed community resources, benefits, and services.
• Work in conjunction with a multidisciplinary team of healthcare professionals to provide comprehensive care, conducting regular home visits to monitor clients’ conditions.
• Act as a mediator in family situations, facilitating conversations to resolve conflicts and ensure a cohesive care approach.
• Educate clients and their families about available programs and resources, enhancing their social engagement and overall vitality.
• Maintain thorough and accurate documentation within electronic health record (EHR) systems to track client assessments, progress notes, and care plan updates.
